Petition Checklist

Please make sure the following elements are part of the petition submitted to the Tribunal of the Diocese of Trenton:

• petition form, completed, with signatures and date and place of the petition
• documents: baptismal (for a baptized person) or birth certificate; marriage certificate for the marriage in question; complete civil annulment or divorce decree
• complete mailing address for the petitioner and respondent (former spouse)
• a list of names and mailing addresses for all witnesses
• a filing fee of $50, payable to Diocese of Trenton
• a summary petition (for notification)
• a "personal history" (where required)
